Today’s baroque church dates back to the 18th century but the cult of the Virgin Mary and the veneration of the miraculous statue started in 1514, with the arrival of the friars of Augustinian order.
The façade portal, divided into two levels and three vertical sets, is located under the blazon with the image of the Virgin Mary with the Infant. Inside the two niches of the upper level are the statues of Saint Augustine and Saint Isidoro Agricolo, made by brothers Palmieri in 1780. Inside there are the wooden statues of Saint Francis di Paola, Angel Gabriel, Guardian Angel and Saint Raphael of 1750, as well as a crucifix. In 1958 the church was officially declared a Marian Sanctuary of the diocese of San Severo.
